Imaging of blood flow, pioneered at Mount Sinai, provides a new way to look into cardiac abnormalities and dysfunction

Researchers from Mount Sinai School of Medicine are pioneering new ultrasound techniques that provide the first characterization of multidirectional blood flow in the heart. By focusing on fluid dynamics specifically, the efficiency with which blood enters and exits the heart's left ventricle the researchers believe they can detect heart disease even when traditional measures show no sign of trouble.

In addition to improving diagnoses, this shift in focus from muscle mechanics to fluid mechanics could lead to more effective therapeutic interventions. The work is described in a study published by two Mount Sinai cardiologists and a team of international collaborators in a recent issue of JACC Cardiovascular Imaging, a journal of the American College of Cardiology.

The ultrasound tools cardiologists use today often fail to detect changes in the heart until there is overt dysfunction. Blood flow imaging, however, may provide better clues in diagnosing heart failure. Sinai investigators reason that flow should be immediately affected by changes in cardiac function such as those revealed in image analysis by the chaotic behavior of tiny whirlpools.

The computer-aided visual study of these abnormalities could dramatically improve the assessment of patients with heart failure and lead to a fresh understanding of normal and abnormal pumping and circulatory function. Visual blood-flow analysis could also yield improved therapies for arrhythmias and other disorders requiring cardiac synchronization. Researchers are actively exploring applications in aortic atherosclerosis, before and after valve replacement, and congenital abnormalities.

"With visualization, we are looking at the ultimate measure of the efficiency of the heart how the blood is brought in and how it is sent out," said Jagat Narula, MD, PhD, Director of Cardiovascular Imaging at Mount Sinai and the senior author of the paper. "Today, cardiologists place great weight on a gauge called the squeeze fraction, or ejection fraction the portion of blood pumped from the ventricle with each heartbeat. What we are doing is a complete departure from the view of the heart as a squeezing, pressure-generating chamber."

Mount Sinai researchers and their collaborators have experimented with a range of imaging techniques to grasp the characteristics of normal and abnormal blood flow. The approaches include phase-encoded MRI, cardiac magnetic resonance (CMR) and several forms of ultrasound-based imaging known as echocardiographic particle imaging velocimetry.

"The most effective technique involves injecting a stream of bubbles that behave exactly like red blood cells and using echocardiography to track their path through the left ventricle," said Partho Sengupta, MD, Director of Cardiac Ultrasound Research at Mount Sinai, and the first author, with Narula, of the JACC paper. In these investigations, the computer-enhanced video output depicts normal and turbulent flow in vivid detail, with arrows plotting the direction as the bubbles swirl through the heart chamber.

"Not only are you following the path of the blood, but you can actually identify the amount of energy that is being distributed," said Dr. Sengupta. "Like other forms of ultrasound, that means low-cost heart tests using this technology could be performed on a simple outpatient basis."

The echocardiography technology pioneered by Sengupta and Narula sheds light on diagnostic discrepancies that have puzzled cardiologists relying on pressure measurements.

"After sustaining significant damage, a patient's heart may not have the greatest squeeze, but there could be good trafficking of blood through the heart and the patient could remain asymptomatic," Sengupta explained. "The normal ejection fraction is around 60 percent, but we sometimes see a patient with 20 percent walking around and playing golf. Other people who are at 50 percent may be short of breath. Flow visualization is one way to capture the essence of why the patient is or is not symptomatic."

Diagnosing cardiac disease by looking for structural defects in the heart is like analyzing highway traffic by examining the road, Dr. Narula said. "The structure may not be great, but how does that affect the cars that are actually traveling on the road? It's the same thing if you fail to look at the blood."

Likewise, a plumber's investigation of pipes in a house only matters or makes sense in relation to how the water flows, claims Sengupta. The flow is multidirectional curling, spinning, and forming eddies that are affected in countless ways by structural changes in heart tissue. As with any new observational techniques, data from novel cardiac visualizations in complex environments are subject to interpretation.

"We have started using these imaging techniques in clinical trials," Narula said. "They will require careful evaluation."

CAIRO (Reuters) - The Muslim Brotherhood and a military man identified with the police state of the ousted Hosni Mubarak courted defeated first-round candidates in Egypt's presidential election on Saturday, each trying to claim the mantle of the revolution for a runoff next month.

The choice between Mursi and Shafiq, representing forces that have tussled for the past six decades, has dismayed many Egyptians who voted for candidates offering a middle ground.

They fear a victory for the 70-year-old Shafiq, Mubarak's last prime minister, would snuff out hopes of change ignited by the anti-Mubarak uprising, while a win for Mursi would pitch Egypt into an experiment with Islamic rule.

The Brotherhood invited rivals, including losing candidates such as leftist Hamdeen Sabahy and moderate Islamist Abdel Moneim Abol Fotouh, to talks on Saturday aimed at gaining their support for the runoff against Shafiq on June 16 and 17.

Warning of "determined efforts to recreate the old regime", it said parties that supported the uprising against Mubarak must unite "so that the revolution is not stolen from us".

The Brotherhood already holds the biggest bloc in parliament after an election completed in January, but has been unable to assert itself against an army-appointed interim government.

Shafiq used strikingly similar language at a news conference as he addressed youth groups that spearheaded last year's popular uprising.

"Your revolution was stolen," he told them. "I pledge to return its fruits to your hands."

Shafiq said that "the clock cannot be turned back", but that he would not let the country "drown in chaos".

Much of his rhetoric indirectly targeted the Brotherhood, playing on fears among Egypt's minority Christians and secular liberals that a Mursi presidency would threaten their freedoms.

"No exclusion of anyone or distancing of anyone," he declared. "Everyone has a right to be a part of this nation."

On Friday he had told Egyptian television that he saw no problem with the idea of a Muslim Brotherhood-led government if he were elected president.

The generals who took over when Mubarak quit on February 11, 2011, have promised to make way for a new president by July 1, formally ending a messy and often bloody political transition.

But the military, which has supplied all Egypt's previous presidents, is keen to keep its privileges and influence in a new order in which the powers of the president, parliament and government are as yet undefined by a rewritten constitution.

Mursi and Shafiq only narrowly topped the vote on Wednesday and Thursday, in which unofficial results showed less than 8 percentage points separating the top four candidates.

Mursi's 25 percent paled in comparison to the parliamentary election, in which the Brotherhood gained nearly half the seats.

A Brotherhood party official, Yasser Ali, said the talks to persuade rivals to join an anti-Shafiq front would discuss the vice presidency and a new coalition government.

"We know we will succeed in uniting behind the initiative to save the nation and complete the revolution," Essam el-Erian, deputy head of the Brotherhood's Freedom and Justice Party, told a news conference on Friday.

Long a thorn in Mubarak's flesh, the Brotherhood has looked increasingly isolated from other parties since his fall, first facing accusations it was slow to join the revolt and then that it acquiesced in the military rule that followed.

Trust in the Brotherhood was further undermined when the group decided to join the presidential race, after previously pledging to stay out. The group, founded in 1928, says it is the target of a vicious smear campaign by its opponents.

Abol Fotouh, who won about 17.6 percent of the vote by the Brotherhood's count, has said he will now back the Islamist group with which he split last year to run for president.

He did not name the Brotherhood, but said he and his supporters would "rise above our political and party differences" and would "stand in a united front against the symbols of corruption and oppression".

Shafiq has won support among Egyptians who see him as the kind of strongman the country needs to end 15 months of political instability, economic failure and rising crime.

His constituency also includes Christians, who form about a 10th of Egypt's 82 million people. They complained of discrimination in Mubarak's day, but are likely to vote for Shafiq in preference to an Islamist.

Mohamed Habib, a former deputy leader of the Brotherhood who left the group last year in protest at its post-uprising policies, said the group should offer vice presidential positions to at least two people from outside the group.

He suggested one could be a Christian - an idea to which Mursi himself has said he is not opposed.

EU running out of time as Greece nears the exit

(AP) ? European leaders insist they want to keep Greece in the eurozone, but are putting off any agreement on how they hope to accomplish that. Greece says it, too, wants to stay in the eurozone, but until after elections it's uncertain whether it can implement the austerity that Europe has set as a condition for doing so.

Essentially, both are playing for time ? about a month. The question is whether financial markets will wait or force their hand.

Concerns that European leaders lack the political will ? and wherewithal ? to tackle the continent's economic problems have worried the markets for weeks. Among the 17 countries that use the euro, seven are in recession. Business confidence is under pressure and banks are feeling the squeeze. The biggest fear is that if Greece cannot be kept in the euro, other larger economies ? like Spain or Portugal ? might face the same fate.

"The breakup of the eurozone will be a disaster. Greece could leave, and others could leave, and this would be a huge financial tsunami," said Dariusz Kowalczyk, senior economist at Credit Agricole CIB in Hong Kong. "Europe is not doing enough, and the market may not wait for them."

Greece has gone through round after round of massive spending cuts and tax hikes to slash its deficit and rein in its debt in exchange for the international bailout loans that help it pay the bills. But the country is now in its fifth year of recession, and many argue it cannot hope for a recovery if it sticks to the deal. And Greeks ? though still keen to remain in the single currency club ? are calling for better terms or, at least, for the pace of austerity to be slowed down.

In a general election this month, neither of Greece's two main parties, both of which support the bailout deal, fared well. Instead, minor parties that are threatening to renege on those commitments saw their popularity surge. A new round of elections is set for June 17.

If the Greeks pick an anti-bailout government the second time round and renege on the terms of the bailout, the flow of funds will be stopped and the country could be forced into a messy exit from the euro bloc as it has no other choice but to print its own currency to pay its way.

That could cause a deeper fracture the euro ? other debt-stricken eurozone members, such as Spain and Portugal, might also fall victim to market fears that they could be next in line for collapse ? and rattle global financial markets.

There had been hope that this week's Brussels summit would have seen a softening in the rest of the eurozone's stance with Greece, extending, for example, the deadline for some of its reforms and cuts.

Some European countries are already hinting that Greece should be given better terms. Both the International Monetary Fund and the Organization for Economic Cooperation and Development, which monitors economic trends in developed economies, also are pushing for the demands on some countries to be eased.

However, as their summit in Brussels broke up early Thursday morning, the leaders failed to offer any reprieve to struggling Greece. Instead they reiterated that they continued to support Greece's eurozone membership ? provided it stuck to the terms of the bailout deal.

Manuel Barroso, President of the EU's executive Commission, told a post-summit press conference: "We stand by Greece. We expect that Greece also stands by its commitments."

European Council President Herman van Rumpoy echoed the sentiment: "Continuing the vital reforms to restore debt sustainability, foster private investment and reinforce its institutions is the best guarantee for a more prosperous future in the euro area."

Some politicians have already begun to factor in a Greek exit, however. In a frank admission that Greece could wind up abandoning the euro, Luxembourg Prime Minister Jean-Claude Juncker told reporters that the eurozone countries "have to consider all kinds of events," but insisted that "the working assumption" was that Greece would remain part of the euro.

The European leaders aim to have more concrete proposals for strengthening the region at their next summit on June 28-29, but before then Greece will have held its elections, by which time Europe's financial system may have already been pushed to the brink.

French investment bank Credit Agricole's research analysts said EU leaders had set the expectations bar very low for the summit "and yet they managed to disappoint nervous markets."

Shares on the Athens Stock Exchange hit a new 22-year low Thursday, closing down 4.53 percent despite gains made elsewhere in Europe.

There are concerns among investors that uncertainty over Greece's future in the eurozone could spark a run on the country's banks, further destabilizing the already-shaky banking system and pushing the country even quicker towards a chaotic exit from the currency.

Greeks have been steadily withdrawing deposits in recent months. Bank of Greece figures show that total household and business deposits stood at about ?165 billion ($207.2 billion) in March 2012, compared to ?235 billion ($295.1 billion) in October 2009, just before the crisis broke. Meanwhile some ?700 million ($879 million) were withdrawn in the week following the inconclusive May 6 election.

The summit also addressed the issue of whether the risk inherent in some countries holding so much debt should be spread across the entire eurozone ? issuing so-called "eurobonds." This would mean every country could borrow funds at the same rate, substantially lowering the costs for the more indebted countries. French President Francois Hollande wants the bonds to finance growth projects and ease concerns about weaker countries' debt, but Germany's Angela Merkel refuses to consider such an option.

"It would have been great news," said Oscar Moreno of Madrid brokerage Renta4. "It would have been good if there had at least been agreement to study it. It would have calmed things down and been a very preliminary first step to the idea of a Eurobond. But we got nothing."

Economists say eurozone leaders have to find another way to increase its defenses than offering bailout after bailout to its debt-stricken memebers. Javier Diaz-Gimenez, a professor of economics at IESE Business School in Madrid, said that the eurozone needs a fundamental redesign.

"The solution has to be some change in design," Diez-Jimenez said. "Something that says, 'OK, these were the things that were missing.

"Just staggering on, that is not going to work. If you keep staggering on, you are eventually going to fall."
